Independence Rehab began in Orem, Utah, in the year 2000 as two separate therapy contracts owned by Darin McSpadden, OTR/L and Jason Crump, PT. A merger in 2004 ultimately included Chad Fullmer, MPT, and Derek White, MPT as owners—and completed the company in its current corporate form.
